When she practice, keep reminding her that she has to stand tall when she pitches. It also might be advisable to have her go to a clinic or camp during the summer or find a personal pitching coach to give her some lessons.

When my oldest daughter started pitching she done the same. i used a string. Tie it to her shorts and the top of her shirt....it will tighten when she has leaned to far forward.teaches them the posture and the feel of how far she can go without throwing off her pitch.

my coach puts a stick or board down her shirt so every time she leans she feels and knows that she is doing everything wrong. so whem comes game time she's nice and tall.

Just keep telling her to have the poise of a dancer when she pitches. Have her do warmups where she doesn't do her windup, and goes through the windmill to the K and releases. This will help keep focus on staying straight.
